As first Step I would like you to try updating the graphics driver so please do the following:
- Uninstall the current driver
- Download the latest driver at the following link:
https://downloadcenter.intel.com/Detail_Desc.aspx?agr=Y&ProdId=3720&DwnldID=23714
Based on the reports you are running an older driver version So please try updating it first. You can check the following document in case you have issue during the driver installation:

Another good step is to try a different cable if possible. You can try with an HDMI or a DVI Single layer cable.
